Patella fractures are classified as either transverse, stellate or vertical. These three categories can be further classified as displaced or nondisplaced. The arterial blood supply to the patella is derived from two systems of vessels from branches of the geniculate arteries. These two systems supply the middle third and apex of the patella. Initial radiographs obtained in our office revealed what appeared to be a non-displaced transverse fracture of the inferior pole of the patella (Fig. 1). An MRI was obtained that confirmed the diagnosis of a patellar stress fracture and ruled out any patella tendon or intra-articular pathology (Fig. 1). The patient was immobilized in a hinged.

ment of displaced transverse fractures of the patella body [2, 3, 7, 8, 14–17]. At our institution, a modified TBW technique involving bending both ends of the K-wires is used regularly. Using this modified technique, Wu et al. had reported an excellent treatment result with smooth fracture union, good recovery of joint motion, and few. dation for transverse patella fractures is figure-of-8 tension band wiring with vertical can-nulated screws.2 Cerclage Wire Fixation Yang et al25 described a titanium cable cerclage wir-.

The diagnosis of a patella fracture comprises of a physical examination, history of the injury and X-ray imaging to determine the nature and severity of the fracture. The X-ray is the most common and widely used diagnostic tool for identification of fractures. Treatment of Patella Fracture. The treatment of a patellar fracture depends upon the. congruity in patients with transverse fracture. Axial views are of limited use except with vertical fracture pat-terns. Advanced imaging techniques such as arthrography, CT, MRI, and bone scans are rarely indicated for isolated acute patellar fractures. Classification Patellar fractures may be classified as displaced (ie, step-off >2 to 3 mm.

A patellar fracture is a break in the patella, or kneecap, the small bone that sits at the front of your knee. Because the patella acts as a shield for your knee joint, it is vulnerable to fracture if you fall directly onto your knee or hit it against the dashboard in a vehicle collision.

Another approach to a transverse fracture is to secure the bones using small screws or small screws and small plates. Comminuted fracture. In some fractures, the top or, more likely, the bottom of the patella is broken into several small pieces.

Regarding patella stress fractures, so far only distal transverse or lateral longitudinal fractures have been reported, but there are no reports of transverse fractures occurring in the proximal patella.

We describe an extremely rare case of transverse stress fracture of proximal patella in a 9-year-old athlete.A 9-year old boy, who. The treatment guidelines for transverse patellar fractures in children are generally the same as those for adults.

Operative treatment is necessary for transverse fractures that show more than 3 mm of diastasis or step-off at the articular surface. 24, Fixation may best be achieved with the use of the modified tension band technique.

Transverse patellar fracture caused by “muscle violence.” (, ). Discharged to rehab from knee surgery in a blue immobilizer brace, my weight-bearing-as-tolerated leg had to do all the work of getting me around; my casted, non-weight bearing foot on the other side was useless.

Nondisplaced fractures are usually closed fractures. Displaced Fracture. If the patella breaks into two or more separate pieces that don’t remain in contact with each other, the break is called a displaced fracture.

This type of fracture may occur in any part of the patella, and the. Transverse patella fracture. These two-part fractures are most often fixed in place using screws or pins and wires and a “figure-of-eight” configuration tension band.

The figure-of-eight band presses the two pieces together. This procedure is best for treating fractures that are located near the center of the patella.

Fracture pieces at the. The reduction of this fracture requires careful planning. The largest fragments are reduced and temporary fixed with K-wires. In frontal/coronal (transverse) fractures, reduction is easier with the knee extended.

Sagittal fractures are more easily reduced with the knee flexed. Patella fracture classification. Patella fracture may be broadly classified under two subheadings, Un-displaced and Displaced fracture. Un-displaced fracture where there is a crack in the patella without any displacement of fractured pieces.

A displaced fracture is a fracture where fractured segments get displaced from each other. It can comminuted fracture (fracture patella get fragmented. A bone scan is a test to look at your fracture and check for infection. You will get a radioactive liquid, called a tracer, through a vein in your arm.

The tracer collects in your bones and pictures are taken. How is a patellar fracture treated. A brace, cast, or splint may be needed.

These supportive devices stop the kneecap from moving and. Introduction. Patellar fractures comprise –% of all types of bone fractures ().Transverse fractures are the most common type of patella fracture, which often cause a functional disability of the knee extensor mechanism ().Surgical treatment is recommended when there is >3 mm of fragment separation or a step-like discontinuity on the articular surface >2 mm ().

patella fractures account for 1% of all skeletal injuries; demographics male to female ; most fractures occur in year olds; Mechanism of injury direct impact injury. occur s from fall or dashboard injury causes failure in compression; indirect eccentric contraction.

occurs from rapid knee flexion against contracted quads muscle. Patella fractures account for approximately 1% of all skeletal injuries and have a bimodal age distribution. 14 In males, the highest incidence of fracture is between the ages of 10 and 19 years.

Non-displaced and displaced fractures are further categorized by location and pattern of fracture. Patellar fracture descriptions by location and pattern may be predictive of the injury mechanism. Several common descriptive patterns of fracture are shown in Figure Pattern descriptors include transverse, vertical, stellate and multifragmented.

Among all fracture types of the patella, transverse fracture of the body is the most common [1,2,3,4].If displaced, it should be repaired surgically to restore the function of extensor mechanism and aid in fracture healing [5, 6].Tension band wiring (TBW) technique, one of the most commonly used surgical method in treating a tension fracture [7,8,9,10], has a sound biomechanical.

Studies in which no transverse fractures or olecranon or patella bones were studies were excluded, as were reviews, case reports, letters to the editor, book chapters and proceedings Result For the olecranon, studies were originally identified; 20 were.

A patellar stress fracture was first described in a German journal by Muller in 4 and in an English journal by Devas in 5 Stress fractures of the patella have been described to be represented by 2 different types; those with a longitudinal fracture of the lateral part and those with a transverse fracture of the distal or middle part.

Stress fractures commonly occur in distance runners, military recruits, and patients with cerebral palsy with knee flexion contractures. 1 Exposures to forces applied in a rhythmic, repetitive, and sub-threshold manner can cause this form of overuse injury, but even in these groups stress fracture of the patella is rare.

Such injury is recognised in patients who have undergone bone-patellar. metabolic bone disease, were skeletally immature, had prior patellar or knee surgery, or had inadequate documentation.

Patella fractures were evaluated on AP and lateral radiographs and categorized as transverse, pole, vertical, or comminuted. Maximum displacement was measured for all fractures using digital calipers using our institution’s. Patella fractures with comminution, osteoporotic bone, and/or previously failed fixation are exceedingly difficult to reduce and fix.

Moreover, the risk of symptomatic constructs and patients that. Patella Fracture: Review of the Literature and Five Case YUDlS EXLER, MA, PT' Patella fractures comprise one percent of all fractures treated in the hospital. They are classified as: 1) transverse, 2) pole, 3) comminuted, and 4) vertical or longitudinal.

The principal cause of such fractures is the direct force involved in a fall, an industrial. As the fractured patella begins to heal, your doctor may recommend a technique called electronic bone stimulation. This painless therapy may help speed the rate of bone healing and takes place in the doctor’s office or at NYU Langone’s Bone Healing Center.

A doctor places a small electrode or electrodes—which are flat discs that conduct electricity—onto the skin near the fractured bone.Challenges include appropriately siting and bending Kirschner wires without damaging surrounding soft tissues. Damage to soft tissues and malposition of metalwork can lead to complications.

We propose three novel techniques for fixation of simple transverse patella fractures to ease application without additional resources to traditional TBW.
